FALL 2025 SPORTS REGISTRATION IS NOW CLOSED. REGISTRATION WILL OPEN AGAIN IN MID JULY

Sports Physicals can only be completed through the school with the school’s Medical Provider.
Athletes registering for sports must have a current sports physical on file in the Nurse’s Office in order to participate in practice/tryouts. Physicals are valid for 12 months from the date of the last exam.
If the sports physical is not conducted within 30 days of the start of the season, an Interval Health History Update is required per NY State Education Department.
Due to this requirement, STEP 7 of the registration will need to be entered and dated within 30 days prior to the start of the Fall Sports season (7/18/25-8/17/25)
Your child will not be cleared until this is completed.
If the online registration was done 30 days before the start of the season (for example in June), please follow the following Instructions for updating step 7 and changing the date:
- Access your account on the athletic registration system
- Click “view” on your child’s fall sports registration (current year)
- Click “step 7” on the left; make any changes (if your child’s medical history has changed since the June registration), to the pre-participation/interval health history athletic form
- Under parent/guardian’s electronic signature and student/athlete’s electronic signature, CHANGE the DATE to the date of entry that is 30 days prior to the start of the season (must be 7/18/25 or later)
- Click save
The Winter and Spring seasons will also require a health history update within 30 days of the start of those seasons per NYSED. Registrations will be open 30 days prior to the start of those seasons, so the information only has to be entered once.
